BASICs are blowing their own trumpet here. They are the doctors who dont want paramedics to be independent prescribers and be able to RSI patients. SECAMB CCPs are a massive step forward in prehospital care.
Ok wow all I can say is Here in Canada, and The States, and as well As Australia, New Zealand, there is no need for Doctors to ride along or respond to calls. The Paramedics especially in Canada for example a Critical Care Paramedic can carry up to 70 Medical Drugs and can do all the stuff necessary to keep a person stabilized.
The limitations placed on paramedics are those defined by medicine. There is no reason that well trained paramedics cannot provide all of the prehospital clinical assessment skills and procedural techniques that are provided by medical practitioners by BASICS. Worst of all, as indicated in the video, this is being done by medicine who have already done "50 hours" at work.
So the delivery of service is low cast and provided by tired under slept medical staff.
What they don't mention is that in other countries such as Australia, New Zealand and parts of America where it is the PARAMEDICS that are successfully providing theses skills full time on Intensive Care ambulances and response cars, removing the need for a physician response.
What they also don't mention is that in continental Europe the reason for physician response is that they do not have a "paramedic" level service. In the majority of europe you get a basic level or doctor.
Having said all that when they are needed in traumatic brain injured patients their skills are invaluable. But like part 2 states paramedics can cope with pretty much most other stuff which can be life threatening such as cardiac arrests, heart attacks (I don't know any BASICS that carry clot busting drugs-certainly not in my area, they don't need to as we usually provide this treatment), anaphylaxis, acute pulmonary oedema etc. Overall it's the anaesthetic skills that really make the difference
Unfortunately what the documentary doesn't state is that not all BASICS/Prehospital DRs can anaesthetise/RSI at the road side. The vast majority are GPs and those that aren't able to provide that level of care will openly tell you it's because they just aren't able to maintain their skill level in order to remain proficient in RSI. The ones that can are usually anaesthetists or A/E DRs with the appropriate training.
Don't be so silly. It just highlights the fact that there are limitations in extremely serious incidents that paramedics cannot deal with that require a doctor. Nothing of the sort that the video implies that paramedics are useless!
