The monster in the video is called The Guy. Mike Wengren's (drummer) little nephew used to see the face and used to be really afraid of him. He used to whisper "That's the guy! That's the guy, isn't it!"

Wikipedia says there was a contest held in 2005 for a official name for The Guy, and the winning name was Morbus, but David confirmed in a interview there was no such thing, and his only true name is The Guy.

David confirming their mascot is just called The Guy:

I've read on different sites that his name is "The Guy", "Mr. Smiley" and "Morbus". Can you set the record straight...what is your mascot's name?!
[laughs] Well, I don't know about "Mr. Smiley". I haven't heard about "Morbus". He's definitely "The Guy"!
