  • Great pathologic description; incorrect pathophysiology. Paget disease is initiated by increased osteoclast activity. The osteoclasts have increased number of nuclei, have nuclear inclusion bodies(of viral origin?), & are hyperactive. Primary abnormality in Paget disease is intense resorption. The osteoclastic resorption causes increased osteoblast activity and disorganized bone. Molecular defects described in Paget disease affect the osteoclast (RANKL/RANK receptor mutations).
