Three workers were killed and 10 others suffered injuries in an explosion at the TMM (Tuah Maju Multifood) Processing Factory in the Industrial Serasa-Muara area on the 30th December 2008.
The explosion happened during maintenance work done on the factory's cold storage cooling system.
The maintenance work was being carried out for the past four days and gases such as ammonia and oxygen were used in testing the cooling system.
The workers, who prior to the blast were carrying out a pressure test for the installation of a 'spiral freezer' refrigeration system ,used oxygen to build up the system's internal pressure which led to a breach in an ammonia tank.
An explosion took place at the factory when the pressure test was conducted. To conduct the test, the workers were filling the refrigerant system with oxygen in order to build up the pressure inside. Pressure testing for such systems should not be carried out in such a way and should be tested using low compression substances (such as water).
