good players will learn to "Graze" shots AKA let shot touch a pixel a few away from their hitbox. Like the Helix Lazers you see early in LEVEL 1. That's they way to score high in the game. That's a reason that there are a bunch a bullets on screen.
I do agree with you that this game is hard. I suggest you try Touhou Project 8 (Imperishable Night) In my opinion it is one of the easier games to play on Normal.

Is it weird that I think 8 is harder? I really think it is. Fourth boss kills me all the time. I did do a practice of the final boss and can do it barely. I have so much trouble on Touhou games. I can only beat 6 with continues, can't beat 7 or 8 don't like 9 haven't played 10 and I'm stuck on stage 5 in 11. Only Touhou game I beat would be the demo for 12 but that's not really beating it.

First Touhou game I played was Iamp and since I'm good at EFZ I'm ok at those two.
