Looks like the chopper took on water before the initial take off attempt. As soon as it got out of ground effect, the pilot realized there was too much weight on board, and eased on the collective, but since the chopper was starting to go down anyway, slammed into the water, taking on even more water. I'd say the pilot by this time was completely panicing, and didn't know what he was doing.

He never got out of ground affect. From the coning it appears it was trying to climb but couldn't. Possibly over weight.
