I'm on the final month of the game on my first play through and I've never encountered death :D Hehe, for one i never stick around on a single floor for too long, I keep switching floors even when I train. And also...
"You have drawn a cursed card. Death draws near"
OH SHT. RUN BETCHES RUN
But whenever I save my game just to look for death he never appears. :Y feck

Its cool how the music intensifies as you proceed through Tartarus. It DOES give you a kind of "OMG the music changed LOOK OUT" feeling.
