Ya so big handles aren't just for injured people as so done already stated they are just plain for better grip and mobility so I hope that answers your question on why fat handles and who uses manual toothbrushes these days anyway lol

Fat handled toothbrushes are great for people who have hand injuries which make small grips much more difficult than they should be.
