I agree sometimes these ideas are overlooked in the process, even by managers who are just trying to get the work out. Visual designers need to understand another dimension to their work, a functional dimension past image and style. A well designed interface visually can be even more powerful if the user is considered more carefully. I haven't yet gotten Norman's newer book "Future Design" but am looking forward to it as I have been a fan of his work and thinking for years.
I've read every Donald Norman book since I first learned about user experience interaction. His process of explaining the simplest ideas (such as flat door plates for pushing, rather than putting pull handles on them) are incredibly intuitive... too bad designers don't always understand these ideas.
