  • How's about the opposite issue, where a patient recently started on FIRST-LINE chemotherapy for STAGE 4 cancer (with expected survival of 2+ years) presents to the hospital for pneumonia but no respiratory distress. Certain hospitalist colleagues of mine have, in this situation, called a palliative care consult and subsequent family meeting with apparent goal to "change the focus" of the patient's course to hospice... which is a premature decision tantamount to malpractice for undertreatment!

    Ex. Doctor "Your 83 y/o demented grandmother has a very large stroke in addition to the heart attack. She is aphasic, dysphagic and hemiplegic. Her EF is 5% w/ severe pul congestion. Can I intubate her?"
