why it can auto fire?

If the extractor wasn't already positioned on the first round in the belt, the first "dud" shot would have been on an empty chamber since no round was stripped from the belt during cocking.
Cocking a second time after the extractor has ridden up onto the first round, will strip and chamber that round.
Excessive headspace caused the case to separate and damaged the weapon, possibly igniting rounds in the feedway above too.

if the bullet would have been stuck in the barrel there would have been alot more damage.
The head space after replacing a barrel IS critical that is what caused this.They did it on purpose

First round fails to fire completely, probably only the primer went off and it was only enough to push the first round down the barrel but it stayed in the barrel. Second round fires, bullet gets stuck against the first bullet already in the barrel, pressure builds up and can't escape anywhere and then KABLOWEY!
Am I right? Is that what happened?
