The Lens on the Gamecube are Dirty. Clean them with rubbing alcohol and a q tip.

try fixing the disk by putting a little bit of toothpaste on a q-tip then starting from the centre and going around the disk. then wash the disk using water off with your finger, and then dry it with a soft cloth. i fixed my gamecube game, and also my friends wii game using this method.
