Actually I think it refers to Annabel killing him at the end of The Emptiness. Cuz this is a prequel to The Emptiness so it's setting up what the story is about.

I think he may refer to the story told by the album. Although based on Dante's Inferno, it still is mostly incorporated into thier story told in the emptiness. Don't mean to spoil or anything, but I believe he is referring to the nightmare he is having, as it seems to be a reocurring cycle, and him 'dying' actually refers to the death of 'Annabel'. I think dying is meant in a metaphorical sense, not a literal sense.
