Intracranial Pressure Patient

I had a request about Neurological Assessments/Treatments with a patient whom had Intracranial Pressure. So I did a quick breakdown of what I remember from Nursing School.

Neuro Assessment:
Important to use GLASGOW COMA SCALE.
-Assess level of consciousness
-PERRLA
-Cushions Triad
-Reflexes
-Oculocephalic Reflex (doll's eye reflex)
-Noxious Stimuli
-Babinski refllex

Neuro Signs/Symptoms:
-Headaches
-Decline LOC or slow response to stimuli
-Slurred speech
-Dilated pupils
-Wide systolic/diastolic pressure
-decreased HR & Respirations
-Projectile vomiting
-Absent reflexes

Treatment:
my acronym is SLAM!!!!!!!
(S) teroids to decrease inflammation
(L) asix to pull fluids from vascular compartment to decrease pressure
(A) lkalosis state for brain vasoconstriction
(M) annitol medication to pull fluid from brain cells to decrease pressure

Other important treatments are to
-Decrease stimuli
-Decrease suctioning
-Avoid restraints
-Avoid coughing, sneezing and nose blowing which may increase pressure.
