@whudson821 He did a backflip under his already opened canopy, so the canopy started to turn and he had to cut away but there was another problem and the main didn't just flew away, so he had to unpack his reserve with his bare hands and got another malfunction but was lucky enough to survive this nightmare :)

AFAIK it was a reserve activation with an Omega harness. A line of the main chute was stuck under a flap of the main-container. So he couldn't get rid of the main chute. After it happened 3 or 4 times in one year there was a modification to be made.
