It seems clear that the monologue is, literally, referring to the way that smoking is, in a way, a wolf in sheep's clothing, that draws us in, and kills us. We know it's dangerous, yet still we smoke for the luxury. The same for drugs. The same for power...and the same for that painting. The scene before this stated that "there's bad people on the streets after the painting" - yet Johnny still has it...he loves the painting's luxury, despite its potentially dangerous consequences. I think. :)

@GIRfan4 "that is why we love the drugs and is also why i cannot give this painting back" when talking about what starts sweet and ends bitter hes obviously talking about herion but when talking about what "starts bitter" (his bitter relationship with his father i.e. Lenny from the beginning) "ends sweet" ( In the end, Lenny getting his legs smashed because he doesnt have the painting, Johnny does) is my take on this scene. hope that makes sense
