The guy who fueled us in Burley, Idaho didn't get the cap tightened down securely. Kathleen noticed fuel streaming from the left wing so we landed at Idaho Falls to check it out. Luckily the cap was still there.
Been there; done that. This same scenario happened with me when flying my Citabria from GA to CO. The guy who refueled my airplane failed to tighten down the cap all the way. I did not double check the caps when he was done......I will not make that mistake again.
There was similar situation on my airport few months ago. I was piloting a hot air balloon and saw an airplane taking off from nearby airport. I noticed fuel stream from the wing, contacted the airplane over the radio and pilot immediately turned back.
The fuel cap is found on ground. Somebody completely forgot to return cap.
It is the pilots responsibility to do a walk around before every flight... and this is the reason why..

@n4117y I think so, but we're all human, hopefully the pilot doesn't forget to check it the same day the line crew guy forgets to secure it properly

Is it not the Pilots responsibility to check his/her own fuel cap after a refuleing???
