This ridiculously cool effect is covered in a Lynda.com video titled "After Effects CS4 Beyond the Basics " by Chad Perkins. No 3D models or multiple stitched shots were used...just a single 2D image cut into segments mapped to displacement greyscales. Actually pretty straightforward....
