Poor tragic Catherine. It was probably Henry's genetic fault that all of their children except Mary died, not Catherine's, since all of her sisters were quite fertile and had numerous healthy children. Henry's only living boy, his son by Jane Seymour lived, but he was a weak child who died early on. Yet, Catherine was blamed for the fact that they did not have sons.

She's wearing a hair shirt, and it tore her skin. Hair shirts were made of rough animal hair and were supposed to be uncomfortable. They worn close to a person's bare skin, as a sign of humility and penance to God. In this scene, KoA is depicted as wearing a hair shirt to appeal to God, in the hopes that He would take pity on her, and give her a son.
