The cogs are driving the pistons, not the other way around. You can tell when it gets to the end of the stroke the surrounding gear structure is driven up or down. This is also probably why there are 2 cog wheels, to help keep it stable.

And this is efficient how? Looks like a mockup of a rotary engine - and a bad one at that. Fact is, rotary engines are very inefficient at low rpms.
