Great discussion and nice collection of stakeholders. How DO you get rid of the middlemen/HMO-gatekeepers and disband the massive infrastructure? We lived in France for a year, had a great GP, had housecalls! and at one point my daughter had emergency surgery - it was SO straightforward and sensible. Having experienced their socialized system I became overtly outraged at our American "system". American health care is a massive scam.
