HeartShare: Community Follow-Up Program

HeartShare provides much-needed services to individuals and families affected by HIV/AIDS. Often, people do not know the full range of services available to them. HeartShare's Community Follow-Up Program helps people to learn about available options and services which will help assist with day-to-day living situations.
