It wasn't. That mother was drugged out of her mind. The "pushing" you speak of didn't come back into existence until the mid-to-late 60s- and at the time was called "Natural Birth" - the first women to use it were looked down upon as freaks and weirdos.
From the late 1800s until the early 60s, all mothers were drugged out of their minds during births, so the doctors invented instruments to pull the babies out. This in turn caused a lot of nerve damage in the babies.

I love how the mother just seems to lay their limp while the doc yanks out the baby. I guess "pushing" wasn't discovered until the 70s.
