Kayako V4 Billing Module

Kayako V4 finally came out, but without the promised billing reports. When they finally released them I was surprised to find that I couldn't even sort by the date that the billing entry was posted (only by ticket date). I waited about three weeks for them to respond to my ticket about it and finally built it myself.

This module will very likely improve, but it's functional and as of right now and I'm thrilled to have an automated way to generate client billing details each month.
