It actually happened. An O - rings between the sections of the solidbooster failed and burned off allowing superheated gases to leak out and punch a hole in the main fuel tank which then blew up. So fake it is not. 7 real people died on that day. 7 families had to realize that they would never be the same again. An NASA had been forewarned by the company supplying the solidboosters that this might happen under frosty conditions - as it indeed was the morning it all happened.

Obviously a MAJOR MALFUNCTION! That's the name of the game. Anyone remembering those TV movies the yanks had in the 1980s about their Jewish American Whiz kids. They made stuff that malfunctioned when it needed the most to actually function. You can have an IQ at 200 and still not get it right. What you need is down to earth people who have practical experience to make things happen the way they should. When 7 peoples lives are at stake, "malfunction" is off the table.
