A video tutorial on how to add an opt-in form or banner to your Facebook Profile or Fan Page. To add an opt-in form or banner follow the steps below.

Step 1) Download the application "My HTML" to your profile or "My HTML for pages" to your Fan Page.
Step 2) Add the application to your profile or fan page by clicking on "Account" and "Application Settings"
Step 3) Add application to your wall and tabs
Step 4) Go up to your tabs and click on the arrow to a pull-down menu and click on "Boxes"
Step 5) Scroll down to locate your application "My HTML"
Step 6) Click on the pencil and "Move to Wall Tab"
