That is for Public Quests, mobs and bosses if you loot them directly drop random. The gear that you grab from a PQ boss is dropped in a separate chest from the sky.

So those shoulders were for DoK? I was under the impression that bosses would drop something you could use, regardless of your class. Does this not apply to world bosses?

Such named heroes are there in each zone, t1-t4, some type unlocks tome and title.

Is a hero, heroes are small Lord opponents as enemies. Heroes have rarely special attacks.
