Atrial Flutter with 4:1 Block (4 P waves to 1 QRS)

P waves are described as "Sharks Teeth," "Sawtooth," or Picket Fences." T waves are not visible. Can also have 2:1, 3:1, or variable blocks.

  • its a ryhthm with the artia creating a loop that discharges pluses at high rates so p waves a flutter for now

  • Rhythm - Regular

    Rate - Around 110 beats per minute

    QRS Duration - Usually normal

    P Wave - Replaced with multiple F (flutter) waves, usually at a ratio of 2:1 (2F - 1QRS) but sometimes 3:1

    P Wave rate - 300 beats per minute

    P-R Interval - Not measurable

    As with SVT the abnormal tissue generating the rapid heart rate is also in the atria, however, the atrioventricular node is not involved in this case.

    Amiodarone can also be used for both A-fib and A-flutter, like we see here. It's not often the "go to" drug, but it's definitely an option.

  • Not really accurate Purpla66, sorry. Flutter is due to re-entry around a circuit usually in right atrium. Usually this gives atrial rate 300bpm, with v rate, untreated usually 150,ie 2:1 block.Only regular if there is consistent av block, often irregular once rate modifying treatment taken. Amiodarone works for flutter as well as AF. Although the US indication for amio may be ventricular arrhythmia, it's often used in Europe for atrial arrhythmia, very effective.

  • it occurs due to a rapid firing of the SA nodes , so the av cannot conduct all these impulses .

    it's ch.chrized by multiple V-shaped p waves or they call it saw tooth waves .

    PR interval not measurable and the atrial rate high reaches to 400\min ,and ventricular rate 60-100\min ..the rhythm is regular ..

    amiodarone's only indication is for ventricular arrhythmias.
