A pressure ulcer and IAD are not the same thing, although they may look similar in early stages.
A pressure ulcer is defined as any lesion caused by unrelieved pressure, resulting in damage of underlying tissue.
IAD is defined as an inflammation of the skin that occurs when urine or stool comes into contact with perineal or perigenital skin.
Essentially, skin damage from pressure ulcers occurs from the inside out. But IAD starts on the surface and works inward.
IAD has many risk factors, including fecal incontinence, compromised mobility, and moisture.
Studies show the odds of having a pressure ulcer are 37.5 times greater for patients or residents who had both impaired mobility and fecal incontinence.
Pressure ulcers are also extremely costly.
The costs of treating a stage 1, 2, or 3 pressure ulcer can range from 2-thousand to 30-thousand dollars....
And a stage 4 pressure ulcer can cost upwards of 70-thousand dollars to treat.
The Centers for Medicare and Medicaid Services, or CMS, is increasing the financial pressure on hospitals by no longer reimbursing for pressure ulcers not present on admission.

To use shield barrier cloths, remove a cloth from either the tub or package.
Communicate to the patient or resident what you are going to do.
Remove the wet or soiled undergarmet, and if needed, gently remove excess fecal material with the underpad, toilet tissue, or wipes.
Remove a shield barrier cloth and cleanse the soiled area.
Use as many cloths as needed to remove urine and/or feces.
Dispose of barrier cloths after use by rolling them inside the underpad or discard according to protocol.
It is extremely important to never flush Comfort Shield Barrier Cloths.
Be sure to remind patients or family members not to flush the barrier cloths as well.

Next, inspect the perineal area for incontinence-associated dermatitis or any skin breakdown.
Report any changes to the RN using the Peri Check guide.
Place a clean, dry garment on the patient or resident, and change any soiled or wet bedding if necessary.
