The Miz received assitance from The Undertaker, and possibly Hornswoggle. Being pre-positioned just by the gateway to the underworld beneath the ring, his wheelchair was lifted up through the gateway, and onto the canvas. Three inches to the left, and he would have plummeted into Hornswoggle's attic.
