Since you are starting from D, the fifth note or 'PA' from D, happens to be the sixth white key.

I love your explanations and presentations and your knowledge on the subject. But, I didn't understand the 'fifth key' part of the explanation because when you are saying 'fifth' you seem to be pressing the sixth white key on the harmonium.
