Probably the best way to keep the formatting unchanged and all the settings in place is to have the text entered into a table. We're going to do a sample here for better understanding. Go to the Insert tab and under the button "Table" select three rows and two columns. For our first row we'll use the "Merge" function. Select both cells and go under Table Layout tab. From there choose the option "Merge Cells". Now we can enter here our text, for an example "Please enter your data here". Into the left in our next rows let's enter "Name" and "E-mail" accordingly. The right handed cells are left empty at this point for users to fill in later. Remember that tables in Word can be extensively tailored to your needs. Now all you have to do is hide the gridlines and no one will tell the difference. Office ToDo - http://www.officetodo.com
