This is a young patient in her 30's who had a severe Z syndrome that was referred to me after Yag capsulotomy did not resolve the problem. She had 5 diopters of lenticular astigmatism and was extremely unhappy with her vision. Vitreous was adherent to the optic through the Yag opening and a decision was made to do pars plana vitrectomy and exchange the lens. A new lens (AR40e) was placed in the capsular bag.
