To reach the "good" ending you need to beat Graham with Flame Dragon, Giant Bat and Succubus Souls equipped, once you beat him you can continue the game to Chaos Realm, the entrance is in Floating Garden, throught the black mist.

If you beat the game with the "good" ending, after beating Chaos you can select hard mode in new game. If you put "JULIUS" as the character name you play with Julius, if you put "NOUSE" you cannot use items, if you put "NOSOUL" you cannot use any soul just the necessary to beat the game. Also you get the chance to play Boss Rush in "Special" menu once you beat Chaos, after beating Graham in this mode Game you will be rewarded with some item according the time you took to kill the bosses.

1 or 2 things sound like secrets but as for other stuff it seems to mainly be stuff only obtainable in hard mode.
