Echo Features of Tamponade: 1) Large volume pericardial effusion; 2) RA and/or Rv diastolic collapse; 3) Doppler features are of exaggerated changes in transmitral and transtricuspid flows and changes in flow patterns of the superior vena cava.

  • Actually the heart typically speeds up in order to try to "keep up" since the amount of blood being pushed out every beat is limited due to the fluid restricting the hearts ability to receive blood during its filling phase. Tachycardia + hypotension.

  • The tamponade is more of a slowing motion of the heart due to the outside pressures?

  • Once it gets to the point of collapsing the right atrium, it's typically a dire situation...apical 4 chamber view is best to see collapse though

  • Don't forget that acute gain of pericardial fluid can put a patient in tamponade. It isn't always a large volume!
