What happened was that Christian realized that Ava was really a man who underwent an operation to become female...she is a trangender female. Ava's vaginal canal is not as long as a natural female's because it was artifically created through surgery; so, when Christian inserted his penis, it stopped short (so to speak) making it painful for Ava. Then, he realizes that she is really a HE...lol.
