There's nothing wrong here, I'd remove everything. Surgeons never remove enough for stupid reasons. It's the same for guys: If your junk has got to go, you might as well make sure the cancer has got no chance of coming back.

It is scary, but this is how they do it in 3rd world countries. Very gruff approach to what should be a meticulous, elegant surgery. Hard to watch, even as a fellow surgeon. If your a paient, don't let this be your image of what breast surgery is.
