Marnie was sick and yes she was given medicine by Nathan but he did not poison it. Rotti secretly poisoned it and Nathan had to save Shilo and let Marni die. Shilo was not actually sick, Nathan told her she was so she would not go outside and get killed. She was the only thing he had left. He gave her medicine that simulated an illness so she would believe it.

That's not true, actually. The 'blood condition' isn't real; her dad's 'medicine' is actually given to her to keep her inside & away from the real world :]
