Have patience...
Put the .map file in your DUKE3D directory and start up the game. Choose USER MAPS in episode selection and then choose the level to play.

Once I have downloaded this file, where do I put it?

Just put it in your Duke3D directory. When you start the game, select Episode Selection, pick User Levels, then go find and pick the level. The game will then play!
