They were not lowering it, just holding it when the brake failed.

He let it slip a little, probably by accident, and then just didn't have the power to stop the momentum. Overloaded? Even the smallest of picks can go wrong if you make a mistake. I once swung completely unloaded, hyd. boom retracted, and nearly vertical. The outrigger had lost its shoe. I tipped, but luckily was so close to a wall that it didn't go far and caused no damage. Just had to change my shorts. Now I get out and check every time.

The explosion is the overheated oil returning to the reservoir and blowing the top off, then caught fire. The official cause is brake drum failure, but you will have that when you let a load of that size drop even a little. Operator error is more likely.

Official cause was operator error, a good mate was the nightshift driver of the white undamaged crane.

A mate was driving the white crane in this vid, he was involved in reaving the main lowering crane, he said there were a lot of weird noises coming from the winch, which were reported, so they knew... The operator was doing all right, down a bit, reverse a bit, down a bit etc. the winch let go, it exploded, bits of it were scattered all over the place. The official cause is operator error, but with the amount of dollars involved, who knows, funny things happen in big business if u get my drift
