  • He let it slip a little, probably by accident, and then just didn't have the power to stop the momentum. Overloaded? Even the smallest of picks can go wrong if you make a mistake. I once swung completely unloaded, hyd. boom retracted, and nearly vertical. The outrigger had lost its shoe. I tipped, but luckily was so close to a wall that it didn't go far and caused no damage. Just had to change my shorts. Now I get out and check every time.

  • The explosion is the overheated oil returning to the reservoir and blowing the top off, then caught fire. The official cause is brake drum failure, but you will have that when you let a load of that size drop even a little. Operator error is more likely.

  • They are going to love me for this...It happened in 2008 at the Westfarmers Curragh coal mine in central Queensland, Australia. The only injury (minor) was the operator of the Lampson 4600 translift that was supporting the dragline boom and incidently dropped it.

  • did the brake fail or did he let it slip and it just took off ? was it overloaded? I am new to cranes and want to learn as much about them as possible. the best way to learn is from other mistakes.
