Nothing. It is just the term. Many times a hook might even drift downstream upside down, depending on how it's tied, the current, etc...
What is important, is that the point of the hook touch flesh, so that it bites and one gets a strikeup. That is what makes beak hooks, egg hooks.. both now called ocopus hooks effective, as they slide through a closed mouth less often than a normal hook.

All my videos are to get ya'll to think outside the box. More than a few offset, kirbed, reversed, true turn models out there, they are just marketed to 10 times the numbers of fishers - bait fishers and artificial plastic lure fishers! Hook makers listen and make what they demand, better "catching" hooks. Lure fishers are keen to experiment to improve their sport, while many (or most) fly fishers have to SEE something in print, or have a "guru" tell them to try something.
