@565coldskill565 The version of them in SH2 (Bubble-Heads) WERE indeed created by James, but look at the designs of the nurses in SH1 and 3. Granted the nurses Heather encounters are similar but they were unique in regards to Heather. Now look at the nurses (and doctors) in the first game, they are entirely different to anything James' warped little mind could think up.
I always believed that the monsters in each game had some psychological relevance to a particular character. :)

@DeadManLimping But that the same with the nurses then, they appear in every single game, when they were created by James, they shouldnt be in all the games, but it is suppose that the otherworld has a "memory" that saves some of the monsters.
