Critical Conversation Advanced Care Planning and Decision Making Models that Work Part 2

On October 7, 2011, the American Academy of Nursing hosted a Raise the Voice Critical Conversation entitled Best Practices in Advanced Care Planning and Decision Making: Models that Work at the Kaiser Family Foundation in Washington, DC. This critical conversation engaged policymakers and other key stakeholders in a discussion of the policy options for promoting best practices in advance care planning and decision-making and encouraged journalists to report on these best practices.
