So how far over the desk or other surface can you hold your hand and still have it track?
Since you are holding your arm up the entire time, is it really less stressful? Can you rest your arm between moves?

You pretty much have to have to bottom of your hand resting in the table surface for it to track movements. The mouse has a little plastic lens on the bottom to focus; I tried it without that and it didn't track consistently at all. I have to put it back on. The nice thing though, is that it will only track when touching a surface, so if you turn your hand sideways (or resting without the lens touching the table), the mouse doesn't jump everywhere.

I'd say the most reduction of stress comes from not needing to twist your wrist (keeps your hand perpendicular to the table, as opposed to parallel with a keyboard, or normal mouse).
Not long ago, I found someone marketing something like this with light weight fabric that only covered part of the hand. I thought they should be in production soon, but I forgot the company that makes it. I'll have to post up a link when I find it again.
