He's breaking the rules by giving it to her so it has to be in a place where no one will see it. Not to mention he's perverted and gets kind of sick thrill from it. When you watch the whole movie you see that hes's giving the patients drugs they want in exchange for sexual favors.He abuses the power he has and he likes doing it.
