Yes, it works over the Atlantic. This video was taken over the Atlantic (on a Frankfurt-JFK flight).

Yes, it works perfectly, but Lufthansa is not permitting Skype (nor is it permitting voice calls when mobile connectivity is offered next year). The carrier says passengers don't want it.
