this is isn't the formal life and death theme. This is the version that plays when Charlie dies, which mixes his theme with the Life and Death theme. It's common in the series. Giacchino mixes a lot of major character's themes with the life and death theme when they die. Charlie was one. Others include Jin and Sun, Juliet, Jack. Also, it's been blended with the Oceanic 6 theme and Main theme.
