You all are over-reacting.
At 0:14, McCain taps Obama to get his attention. Obama turns around and assumes McCain wants to shake his hand, but McCain directs him to Mrs. McCain. Obviously, McCain was tapping Obama so he could shake his wife's hand. Then at 0:17, McCain realizes that Obama was going to shake his hand, so he extends his own hand and makes some sort of joke. Both men then smile.
