EDIT: Well, apparently the music in V1 broke the copyright laws and got muted, so i had to reupload it with somethin' a tad different.
It had a lot of great comments (Which unfortunately reset due to the reupload), thanks a bunch :] I'm really glad people find this useful.
The first things you must do in order for this to work, is download a hex editor. I'm using XVI32 in this tutorial
XVI32 Download:

Then, you need something to turn the decimals into hex numbers.
Here's a little something to help you out:

A little something i stitched together 'cuz i was bored, hope it's not too difficult to understand. You'll probably need to pause the video every now and then, for the tempo can be rather fast at times.
So, have fun with your bloodfang warlock dual wielding Cataclysm's edges, and grand marshal hunters wielding a gun and a sword at the same time. The possibilities are limitless ;]
Just a quick note, you can't put pants into your head. Believe me, i tried :D Nor can you put a shield into your main hand, since it'd be the wrong way. Same goes for main hand fist weapons for offhand and the other way 'round. Oh, and the edits reset everytime you patch, so if you wanna keep your edits, just move the itemcache into some other folder while you patch.
Q&A so far:
Q: Can skin and armor can be changed this way too?
- A: Yes, armor changes work just like weapons. Skins are a bit different and i have no experience from that, sorry : /
Q: can you get banned for this?
- A: No, Blizz has no way of seeing what you have done with your itemcache. There are some programs which do the same thing itemcache tunin' does, and some of them can be tracked. But not this, nu-uh.
Q: Does this still work in 3.1?
- A: Yeah, it should still work, does for me at least.
Q: Can other people can see the changes you make?
- A: No, it's just you.
Q: Can mounts be edited too?
- A: Umm, i thiiiink soo... probably. But i havn't looked into it, so... Google is your friend. Or maybe if i get bored i'll do a second guide with some more stuffs ;o
Q: Changing the sheathing of my staff fail'd. Halp!
- A: The number to wield a weapon in your back like a staff is 02
Q: How to reset the changes?
- A: Just delete the itemcache, WoW will create a blank new one.
Yours truly
Tahna
Hey it works perfectly, but i have a problem. I changed my rogue's "Venerable Mass of McGowan" with "Warglaive of Azzinoth" but i held them on my waist which is kinda.. not cool ;/ and since heirlooms got no durability how could i change the place they are worn? Sorry about my bad english.

@Lloyd4o The easiest way that comes to mind would be to make a backup of your itemcache and just look for a random 03 number (waist) from somewhere between your weapon and the next piece of equipment in the editor and try it out. If it doesn't work, just create another copy of the file and try again. Don't worry, there shouldn't be too many 03s around. On the other hand, it HAS been several years since i last tried this out myself, so... If it doesn't work, we'll try to think of something else
